7 Warning Signs That L4–L5 Lower Back Pain Is Becoming Serious

Pain at the L4–L5 level of the lumbar spine is common, but sometimes it can indicate a more serious nerve or disc problem. If certain symptoms appear or worsen, medical attention may be necessary.

Below are 7 important warning signs that L4–L5 pain may be becoming serious.

1. Severe Pain Radiating Down the Leg

If the pain spreads from the lower back to the buttock, thigh, calf, or foot, it may indicate sciatic nerve compression.

Common signs:

  • Sharp or electric shock–like pain

  • Burning sensation in the leg

  • Pain worsening when sitting or coughing

This often suggests disc herniation at L4–L5.

2. Numbness or Tingling in the Leg or Foot

Persistent numbness or “pins and needles” in areas such as:

  • Lower leg

  • Top of the foot

  • Big toe

This can mean the nerve root is irritated or compressed.

3. Weakness in the Leg or Foot

If you notice:

  • Difficulty lifting the foot

  • Trouble climbing stairs

  • Weakness while walking

This could be nerve damage affecting muscle control.
